The PCN offers a range of primary care services to patients, providing access to clinical disciplines in individual practices which allow GP practices to proactively and holistically care for patients and encourage them to manage their own health and well-being. With increased demand, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.
