Changes to how patients make appointments from Monday 1st July 2024

Dear Patient,

To deal with the increasing demand for appointments the Practice is following the lead of many other Leeds Practices and changing the way you access our services.

All requests for the surgery need to be made via the forms on our website or the NHS app, whether this be for an appointment, repeat prescription, sick note or general query. For those patients unable to access this form, you can still call the surgery and a receptionist will complete the form with you.

All these forms will be accessed by the medical team who will direct your query to the most appropriate healthcare professional or service.

This service is available in core practice hours Monday to Friday 8am – 6pm. Queries outside of this time should be directed to 111. Life threatening emergencies should be dealt with by dialling 999.
